ABSTRACT

ABSTRACT Objective: To assess the role of radiological predictive markers on orthopantomogram for inferior alveolar nerve (IAN) injury related to the removal of mandibular third molar surgery and the occurrence of post-operative IAN paresthesia. Material and Methods: This prospective observational study was conducted on 60 patients (aged 17-35 years) indicated for extraction and showed one or more of the seven previously known panoramic radiographic risk signs of IAN injury. Variables such as age, sex, tooth angulation, and relationship with the inferior alveolar canal (IAC) were assessed to see their outcome on IAN injury. Data analysis is presented through tables and descriptive methods. Results: Among patients, 26 were male and 34 were female, with a mean age of 26.17 years. Out of seven radiological predictive markers, only six were found in this study, whereas one marker, viz. interruption of white line of the canal was not found. After surgical removal of the lower third molar, only two patients with radiographic signs showing the deflection of roots and darkening of roots continued with sensory deficit 5 weeks post-operatively. Conclusion: The risk of inferior alveolar nerve injury during lower third molar surgery is very low, even in patients with radiological predictive markers.

Introducción: El desarrollo de las ciencias médicas trae consigo un incremento en la expectativa de vida, junto a la detección temprana de un gran número de enfermedades crónicas como las cerebrovasculares y cardiovasculares, que son tratadas rutinariamente con medicamentos antiagregantes plaquetarios. El conocimiento del manejo de estos pacientes ante los procedimientos quirúrgicos estomatológicos constituye un reto en la práctica diaria profesional. Objetivo: Determinar el nivel de sangramiento posextracción dentaria en pacientes con enfermedad cardiovascular y cerebrovascular, según el tipo de antiagregantes plaquetarios y grupo dentario, así como la frecuencia de utilización de las medidas para su control. Métodos: Se efectuó un estudio observacional, descriptivo, longitudinal y prospectivo con un universo de 136 pacientes de más de 20 años, remitidos por su cardiólogo y que necesitaban realizarse extracciones dentarias sin modificar su tratamiento con antiagregantes plaquetarios. Las variables estudiadas fueron la enfermedad sistémica, el tipo de antiagregante plaquetario, el nivel de sangramiento, grupo dentario intervenido y método hemostático utilizado. Resultados: La mitad de los pacientes estudiados no presentó sangramiento posextracción dentaria. En los pacientes tratados con aspirina o clopidogrel predominaron los sujetos sin sangramiento para un 84,3 por ciento y 62,5 por ciento, respectivamente. En los de doble antiagregación prevaleció el sangramiento moderado con un 46,3 por ciento. Los grupos dentarios incisivo, canino y premolar no presentaron episodios de sangramiento para un 64,1 por ciento, 51,6 por ciento y 53,3por ciento, respectivamente. El método hemostático más utilizado fue la compresión de las corticales y termoterapia fría (47,8 por ciento). Conclusiones: La mitad de los pacientes con enfermedades cardiovasculares y cerebrovasculares no presentaron sangramiento posextracción dentaria(AU)


Introduction: The development of medical sciences brings with it an increase in life expectancy, together with the early detection of a large number of chronic diseases such as cerebrovascular and cardiovascular diseases, which are routinely treated with antiplatelet aggregation drugs. Knowledge on the treatment of these patients before stomatological surgical procedures constitutes a challenge in daily professional practice. Objective: To determine the level of bleeding after tooth extraction in patients with cardiovascular and cerebrovascular disease, according to the type of antiplatelet agents and dental group, as well as the frequency of use of measures for their control. Methods: An observational, descriptive, longitudinal and prospective study was carried out with a universe of 136 patients over 20 years of age, referred by their cardiologist, who needed dental extractions without modifying their treatment with antiplatelet agents. The variables studied were systemic disease, type of antiplatelet agent, level of bleeding, dental group treated and hemostatic method used. Results: Half of the patients studied did not present bleeding after tooth extraction. In patients treated with aspirin or clopidogrel, 84.3 percent and 62.5por ciento, respectively, had no bleeding. In those with double antiplatelet therapy, modera te bleeding prevailed with 46.3 The incisor, canine and premolar tooth groups did not present bleeding episodes (64.1 percent, 51.6and 53.3 percent respectively). The most commonly used hemostatic method was cortical compression and cold thermotherapy (47.8%). Conclusions: Half of the patients with cardiovascular and cerebrovascular diseases did not present bleeding after tooth extraction(AU)

Objetivo: La fibrosis periapical posendodóntica es un proceso reparativo asintomático, radiolúcido y no progresivo que se interpreta con frecuencia como una lesión patológica persistente. El diagnóstico de esta entidad suele ser dudoso y sólo puede definirse mediante la correlación de las obser- vaciones clínicas, radiográficas e histológicas. El objetivo de este informe es describir el caso de un paciente que presenta un área radiolúcida periapical persistente y asintomática en un incisivo lateral superior. Caso clínico: Luego de cuatro años y dos meses de ha- ber recibido un tratamiento endodóntico el paciente concurre a la consulta para un examen de rutina. El examen radiográfico del diente revela un área radiolúcida persistente y bien defini- da. A causa de una fractura radicular irreparable, se indicó la extracción de la pieza dentaria. Esto permitió realizar el estu- dio histológico del tejido blando que permanecía adherido en el ápice de la raíz, lo que confirmó y completó el diagnóstico de fibrosis periapical posendodóntica.(AU)


Aim: The postendodontic periapical fibrosis is an asymp- tomatic radiolucent and non-progressive healing process that is often interpreted as a persistent pathological lesion. The diagnosis of this entity is usually uncertain, and it is only de- fined by the correlation of clinical, radiographic and histo- logical observations.The aim of this report is to describe the case of a patient with a long-term persisting asymptomatic and radiolucent area in a upper lateral incisor. Clinical case: Four years and two months after receiv- ing an endodontic treatment the patient comes to our office for a routine control. Radiographic examination revealed the presence of a persistent well defined radiolucent area. Sur- gical tooth extraction was required due to the presence of a complicated root fracture.This allowed to perform a histolog- ical study of the soft tissue attached to the apex of the ex- tracted root, which confirmed and completed the diagnosis of postendodontic periapical fibrosis (AU)

ABSTRACT

Introducción: La administración de bifosfonatos y medicamentos antiangiogénicos en pacientes con cáncer es un esquema terapéutico usual en oncología. Existen reportes de osteonecrosis de los maxilares en pacientes sometidos a este esquema de tratamiento, luego de realizar un procedimiento dental invasivo. Objetivo: A partir de las características clínicas e imagenológicas de la patología, ilustrar al odontólogo sobre los medicamentos para el tratamiento del cáncer, susceptibles de generar osteonecrosis de los maxilares. Presentación de caso: Paciente masculino de 89 años, con cáncer de próstata tratado con denosumab, que desarrolló osteonecrosis del maxilar inferior posterior a una extracción dental. Es de vital importancia que el odontólogo identifique los medicamentos, factores de riesgo y las medidas para minimizar el riesgo de osteonecrosis de los maxilares en pacientes susceptibles(AU)


Introduction: The administration of bisphosphonates and antiangiogenic drugs in cancer patients is a usual therapeutic scheme in oncology. There are reports of osteonecrosis of the jaws in patients undergoing this treatment scheme, after performing an invasive dental procedure. Objective: Show the dentist from the clinical and imaging characteristics of the pathology on the drugs for the treatment of cancer sensitivity to generate osteonecrosis of the jaws. Case presentation: An 89-year-old male patient with prostate cancer treated with denosumab developed osteonecrosis of the lower jaw after tooth extraction. It is vitally important that the dentist identifies medications, risk factors and measures to minimize the risk of osteonecrosis of the jaws in sensitivy patients(AU)

ABSTRACT

Dentes supranumerários são caracterizados como um excesso no número de dentes da série normal. Os mesiodentes, definidos pela sua localização mediana aos incisivos centrais superiores, referem-se ao tipo de dente supranumerário mais comumente encontrado na cavidade oral, normalmente intraósseo e assintomático. A presença desses dentes pode ocasionar alterações na dentição permanente, mas a abordagem cirúrgica na fase da dentição mista mostra resultados satisfatórios no alinhamento dental espontâneo. Em pacientes pediátricos, a sedação consciente com óxido nitroso tem demonstrado ser uma alternativa segura e eficaz no controle do medo e da ansiedade. Objetivo: relatar um procedimento cirúrgico de remoção de dois mesiodentes em paciente pediátrico sob anestesia local associada à sedação com óxido nitroso. Relato de caso: paciente do sexo feminino, 11 anos de idade, necessitava de remoção cirúrgica de dois dentes supranumerários localizados em região anterior de maxila. Com a avaliação da tomografia computadorizada, pôde-se observar a posição em que ambos se encontravam. Realizou-se também a exodontia do elemento dentário 14, como solicitado pela ortodontista, para posterior continuidade do tratamento ortodôntico. Devido à complexidade cirúrgica e ao tempo operatório, considerando a idade da paciente e o possível aumento nos níveis de ansiedade, optou-se pela realização de sedação consciente com óxido nitroso. Conclusão: clinicamente, dentes supranumerários podem causar danos locais. O tratamento cirúrgico associado à sedação consciente mostrou-se bastante seguro e efetivo no controle comportamental, além de poder ser implementado na rotina ambulatorial.(AU)


Supernumerary teeth are defined as those in addition to the normal series. The mesiodens, is the most common supernumerary tooth and it is present in the midline between the two central incisors. It is usually intraosseous and asymptomatic. The presence of these teeth may cause changes in permanent dentition and the surgical approach in the mixed dentition phase shows satisfactory results in spontaneous dental alignment. In pediatric patients, conscious sedation with nitrous oxide has been shown to be a safe and effective alternative in controlling fear and anxiety. Objective. To report a surgical procedure for the removal of two mesiodens in a pediatric patient under local anesthesia associated with nitrous oxide sedation. Case report. 11-year-old female patient required surgical removal of two upper mesiodens. The computed tomography images assisted to indicate their exact position. The extraction of the upper right premolar (tooth #14) was also performed as requested by the orthodontist. Considering the patient's age and the possible increase in anxiety levels, along with surgical complexity and operative time, the conscious sedation with nitrous oxide was selected. Conclusion. Clinically, supernumerary teeth cause local damage and surgical treatment associated with conscious sedation has proved to be quite safe and effective in the behavioral control besides being able to be implemented in the outpatient routine.(AU)

ABSTRACT

En este informe de caso clínico se muestra el tratamiento exitoso de un paciente con anodoncia parcial de órganos dentales 13 y 23 debido a que fueron extraídos por presentar impactación y mal pronóstico de tracción. En este artículo se presenta un paciente masculino de 29 años clase I esquelética con un perfil convexo, maloclusión de clase III, overjet disminuido, ausencia de guías funcionales, discrepancia oseodentaria positiva en arcada superior y negativa en arcada inferior. El tratamiento se realizó con extracciones de los órganos dentales 34 y 44 para nivelar las discrepancias óseo dentarias interarcadas, se llevó a cabo mediante un cierre de espacios recíproco por medio de cadenas elásticas para ambas arcadas, con lo que se logró crear un overjet y overbite adecuados. La creación de las guías caninas funcionales se consiguió mediante el cambio de morfología de los órganos dentales 14 y 24, los cuales fueron llevados a la posición de los caninos ausentes. El tiempo total de tratamiento para este paciente fue de 24 meses. Se realizaron ameloplastias positivas, la aplicación de agregados de resina para mejorar la funcionalidad y proveer salud articular. Se sugiere que ante casos de anodoncia de caninos en la arcada superior, un tratamiento favorable se puede llevar a cabo mediante el cierre de espacios, la caracterización morfológica de los caninos ausentes mediante ameloplastias positivas en premolares (AU)


This case report shows the successful treatment of a patient with partial anodontia of dental organs 13 and 23 because they have been extracted due to present impactation and poor traction prognosis. This article presents a 29-year-old male class I skeletal patient with convex profile, class III malocclusion, overjet reduction, absence of functional guidance, positive bone-teeth discrepancy in the upper arch and negative in the lower arch. The treatment was carried out with the extractions of the dental organs 34 and 44 for correcting oral dental discrepancies between upper and lower arches; it was done using reciprocal closing of spaces by using elastic chains for both arches in order to achieve a suitable overjet and overbite. The creation of the canine guides was achieved by changing the morphology of the dental organs 14 and 24, which were taken to the position of the absent canines. The total treatment timing for this patient was 24 months. Positive ameloplasties were performed by application of resin aggregates to improve functionality and provide joint health. A favorable treatment for these kinds of cases of canine anodontics in the upper arch is carried out by closing spaces and the morphological characterization of the absent canines by positive in-premolar ameloplasties (AU)

ABSTRACT

Objetivo: relatar um caso clínico de coronectomia com acompanhamento pós-operatório de 15 meses. Relatode caso: a coronectomia é uma alternativa de tratamento para terceiros molares inferiores (3MI) impactadosque apresentam proximidade com o canal mandibular, minimizando o risco de morbidade neurossensorial.A técnica consiste em remover apenas a coroa dentária (na junção amelocementária) com retençãointencional das raízes e da polpa, evitando o dano direto ou indireto ao nervo alveolar inferior (NAI). Oprocedimento foi realizado em paciente adulto do gênero masculino com 3MI impactados. A radiografia panorâmicamostrava o escurecimento dos ápices radiculares, sugerindo íntima relação das raízes com o canalmandibular. No acompanhamento pós-operatório imediato, o paciente retornou sem sintomatologia dolorosa,sem edema facial significativo, apresentando boa cicatrização e sem relato de perda de sensibilidade nemde disfunção mastigatória. No acompanhamento radiográfico, foi verificada neoformação óssea adjacenteaos remanescentes radiculares. Considerações finais: a coronectomia mostrou-se um procedimento simples,eficaz e seguro para diminuir o risco de injúria ao NAI durante o tratamento dos 3MI.(AU)


Objective: to report a case of coronectomy with follow-up of 15 months. Case report: coronectomy is an alternative treatment for impacted lower third molars (3ML) which are close to the mandibular canal, minimizing the risk of sensorineural morbidity. The technique consists of removing only the dental crown (at the cementoenamel junction) with intentional retention of the roots and pulp, avoiding direct or indirect damage to the inferior alveolar nerve (IAN). The procedure was performed on an adult male patient with impacted 3ML. Panoramic radiograph showed darkening of the roots, suggesting a close relationship between the roots and the mandibular canal. At immediate postoperative follow-up, the patient returned with no painful symptoms, no significant facial edema, good healing, and no loss of sensation or masticatory dysfunction. Radiographic follow-up showed bone neoformation adjacent to the root remnants. Final considerations: coronectomy has been shown to be a simple, effective and safe procedure to decrease the risk of injury to IAN during 3MI treatment.(AU)

ABSTRACT Pain control is essential in dental practice, and can be accomplished through various techniques. This study seeks to compare atraumatic and conventional anesthetic techniques, applied during surgeries to remove upper third molars. The endpoints evaluated were pain indices, patient satisfaction and anesthetic efficacy. A random parallel split-mouth clinical trial was conducted with 14 patients. Group A received atraumatic anesthesia without a needle (Comfort-in®) and group B received conventional anesthesia by blocking the posterior superior alveolar nerve (PSAN) and Greater Palatine Nerve (GPN). A Visual Analogue Scale (VAS) was used to assess pain. A significantly (p<0.001) lower perception ofpain was observed among individuals who received the atraumatic technique. In 71% of cases, it was necessary to supplement the anesthesia during the procedure. Even considering the need for additional anesthesia, the Comfort-in® technique was more accepted by patients with regard to pain perception than the conventional manual technique.


RESUMO O controle da dor é fundamental na prática odontológica, po-dendo ser feito a partir de várias técnicas. Este estudo visa comparar as técnicas anestésicas atraumática e convencional, aplicadas durante cirurgias para remover terceiros molares superiores. Os pontos avaliados foram índice de dor, satisfaqao do paciente e eficácia anestésica. Um ensaio clínico randomi-zado, paralelo, boca dividida, foi conduzido com 14 participantes. O grupo A recebeu anestesia atraumática sem agulha (Comfort-in®) e o grupo B anestesia convencional por meio de bloqueio do nervo alveolar superior posterior (NASP) e nervo palatino maior (NPM). Uma Escala Visual Analógica (EVA) foi utilizada para avaliar a dor. Uma significativa (p<0,001) menor percepção de dor entre os individuos que receberam a técnica atraumática foi observada. Em 71% dos casos, fez-se necessària a complementação da anestesia durante a realiza-ção do procedimento. Mesmo considerando a necessidade de anestesia adicional, a técnica empregando o sistema Comfort-in® teve maior aceitação dos pacientes quando comparado à técnica manual convencional, no que se refere a percepção de dor, ainda que considerando a necessidade de complementação da anestesia.

INTRODUCCIÓN: La coronectomía se considera una alternativa a la extracción total en los casos donde las raíces de los terceros molares inferiores están en íntimo contacto con el nervio alveolar inferior. Existe incertidumbre sobre si la coronectomía disminuye la incidencia de parestesia en comparación a la extracción total. MÉTODOS: Realizamos una búsqueda en Epistemonikos, la mayor base de datos de revisiones sistemáticas en salud, la cual es mantenida mediante el cribado de múltiples fuentes de información, incluyendo MEDLINE, EMBASE, Cochrane, entre otras. Extrajimos los datos desde las revisiones identificadas, analizamos los datos de los estudios primarios, realizamos un meta análisis y preparamos una tabla de resumen de los resultados utilizando el método GRADE. RESULTADOS Y CONCLUSIONES: Identificamos seis revisiones sistemáticas que en conjunto incluyeron 17 estudios primarios, de los cuales, dos corresponden a ensayos clínicos aleatorizados. Concluímos que la coronectomía en comparación a la extracción total de los terceros molares inferiores probablemente se asocia a un menor riesgo de parestesia. Además, no está claro si la coronectomía en comparación a la extracción total de terceros molares inferiores podría aumentar el riesgo de infección (certeza de la evidencia es muy baja).

Abstract This retrospective study evaluated facial profile pleasantness determined by two protocols of Class II treatment. The sample comprised facial profile silhouettes obtained retrospectively from the pretreatment (T1) and posttreatment (T2) cephalograms of 60 patients (42 males and 18 females) divided into two groups. One group of 30 patients (mean age of 12.84 years) was treated with the extraction of maxillary first premolars (mean treatment time of 2.7 years), and the other group of 30 patients (mean age of 12.81 years) was treated with a mandibular advancement appliance (Forsus) (mean treatment time of 2.49 years). The facial profile silhouettes (T1 and T2) were randomly distributed in an album containing one patient per sheet. The examiners consisted of 60 orthodontists and 60 lay individuals, who analyzed the profiles in regard to facial pleasantness, using the Likert scale. A comparison between stages T1 and T2 of the two treatment protocols and between the examiners was performed by mixed-design analysis of variance at a significance level of 5%. The results demonstrated a significant difference between T1 and T2 (greater scores for T2 compared to T1), and between lay individuals and orthodontists (orthodontists assigned higher scores), but with no significant difference between the treatment protocols. Both protocols produced positive effects on the facial profile esthetics, from the standpoint of lay individuals and orthodontists.

Abstract Objective To investigate the effects of intro-oral injection of parathyroid hormone (PTH) on tooth extraction wound healing in hyperglycemic rats. Methodology 60 male Sprague-Dawley rats were randomly divided into the normal group (n=30) and DM group (n=30). Type 1 diabetes mellitus (DM) was induced by streptozotocin. After extracting the left first molar of all rats, each group was further divided into 3 subgroups (n=10 per subgroup), receiving the administration of intermittent PTH, continuous PTH and saline (control), respectively. The intermittent-PTH group received intra-oral injection of PTH three times per week for two weeks. A thermosensitive controlled-release hydrogel was synthesized for continuous-PTH administration. The serum chemistry was determined to evaluate the systemic condition. All animals were sacrificed after 14 days. Micro-computed tomography (Micro-CT) and histological analyses were used to evaluate the healing of extraction sockets. Results The level of serum glucose in the DM groups was significantly higher than that in the non-DM groups (p<0.05); the level of serum calcium was similar in all groups (p>0.05). Micro-CT analysis showed that the DM group had a significantly lower alveolar bone trabecular number (Tb.N) and higher trabecular separation (Tb.Sp) than the normal group (p<0.05). The histological analyses showed that no significant difference in the amount of new bone (hard tissue) formation was found between the PTH and non-PTH groups (p>0.05). Conclusions Bone formation in the extraction socket of the type 1 diabetic rats was reduced. PTH did not improve the healing of hard and soft tissues. The different PTH administration regimes (continuous vs. intermittent) had similar effect on tissue healing. These results demonstrated that the metabolic characteristics of the hyperglycemic rats produced a condition that was unable to respond to PTH treatment.

ABSTRACT

Introduction: establish an association between the radiographic classification and the surgical technique related to the lower third molars. Materials and method: a retrospective study was carried out, with the descriptive analysis of the data. The sample consisted of 100 patients (n = 100) from the spontaneous demand who sought out the Dental Clinic of UFC (Ceará's Federal University) ­ Campus Sobral, between december 2017 to july 2018, and who followed the inclusion criteria: patient with panoramic radiography, free of active periodontal disease and tooth with at least two thirds of root formation; and as exclusion criteria: patients with associated bone pathologies, teeth with less than two thirds of root formation and patients who did not wish to participate in the study. Results: patients included in the study were from 18 to 25 years of age, most of them male, with complaints of pain. Using the radiographic classification of Pell & Gregory, the most found positions were 1A (35%), 2B (28%) and 2A (17%). The most prevalent surgical techniques were the open ones. Could be done the extraction by closed surgical technique (forceps or lever) (n = 27), with flap preparation plus osteotomy (OST) (n = 25) and flap plus osteotomy plus odontostomy (ODS) (n = 48). Conclusion: teeth with radiographic classification, grade of inclusion and different preoperative plans had the same protocol (flap + OST + ODS) in the surgical act. It is evident that radiographic classification stills an effective method to aid in operative planning, but it can be complemented by other diagnostic standards, such as a specific classification of the root anatomy. Objetivo: realizar uma associação entre a classificação radiográfica e a técnica cirúrgica relacionada aos terceiros molares inferiores. Materiais e método: foi realizado um estudo retrospectivo, com a análise descritiva dos dados. A amostra foi constituída de 100 pacientes (n = 100), provenientes de demanda espontânea, que procuraram o ambulatório de Odontologia da Universidade Federal do Ceará (UFC) ­ Campus Sobral, no período de dezembro de 2017 a julho de 2018, e que seguiram os seguintes critérios de inclusão: presença de radiografia panorâmica, ausência de doença periodontal ativa e dentes com no mínimo dois terços de formação radicular. Já os critérios de exclusão foram: pacientes com patologias ósseas associadas aos terceiros molares inferiores, dentes com menos de dois terços de formação radicular e pacientes que não desejassem participar do estudo. Resultados: os pacientes incluídos no estudo tinham idades entre 18 e 25 anos, a maioria do sexo masculino, com queixas álgicas. Em relação à classificação de Pell & Gregory, as posições mais encontradas foram 1A (35%), 2B (28%) e 2A (17%). As técnicas cirúrgicas mais prevalentes foram as abertas (73%). Em relação às técnicas cirúrgicas, foram encontradas: técnica cirúrgica fechada (fórceps ou alavanca) (n = 27), com confecção de retalho mais osteotomia (OST) (n = 25) e por retalho mais osteotomia e odontossecção (ODS) (n = 48). Conclusão: é evidente que a classificação radiográfica ainda é um método eficaz para auxiliar no planejamento operatório, mas pode ser complementada por outros padrões de diagnóstico, como uma classificação específica da anatomia radicular. Os caninos maxilares e mandibulares são dentes importantes dos pontos de vista estético e funcional. Esses dentes são frequentemente mal posicionados, no entanto, a falha da irrupção do canino mandibular é pouco comum, sendo mais frequente a inclusão de caninos maxilares. Fatores como posicionamento dentário, anquiloses, cistos, idade do paciente, saúde periodontal, formação radicular, entre outros, direcionam o tratamento. Objetivo: reportar um caso clínico de um jovem com impactação dentária de um canino inferior direito, enfatizando e discutindo a técnica cirúrgica e o planejamento realizado. Relato de caso: paciente de 15 anos de idade, do gênero masculino, com a não erupção do canino inferior direito, ao exame clínico, apresentou ausência clínica do dente 43, persistência do dente 83. Foi solicitada uma tomografia computadorizada de feixe cônico (cone beam) da região, na qual se diagnosticou a presença do elemento dentário 43 em posição horizontal localizado no mento. O plano de tratamento proposto foi remoção cirúrgica do dente não irrompido. Considerações finais: a opção de tratamento de dentes não erupcionados dependente de fatores como rizogênese incompleta, idade do paciente e grau de inclusão, e as manobras referentes aos dentes envolvidos devem ser realizadas no momento mais oportuno e com adequado planejamento, evitando lesões nas estruturas nobres e permitindo, assim, um processo de reparo adequado.(AU)


Introduction: Maxillary and mandibular canines are important teeth from the aesthetic and functional points of view. These teeth are often poorly positioned but the eruption failure of the mandibular canine is uncommon, and the inclusion of maxillary canines occurs more frequently. Factors such as dental positioning, ankyloses, cysts, patient age, periodontal health, root formation, among others guide the treatment. Objective: To report a clinical case of a young man with tooth impaction of a right lower canine, emphasizing and discussing the surgical technique and planning performed. Case report: A 15-year-old male patient, with non-eruption of the right lower canine on the clinical examination, presented a clinical absence of tooth 43 and persistence of tooth 83. A cone-beam computed tomography was requested for the region in which the presence of dental element 43 was diagnosed, in the horizontal position located in the chin. The treatment plan proposed was surgical removal of the non-erupted tooth. Final considerations: The treatment option of non-erupted teeth depends on factors such as incomplete rhizogenesis, patient age, and degree of inclusion, and the teeth involved should be managed at the optimal time and with adequate planning, preventing injuries to noble structures, thus allowing an adequate repair process.(AU)

ABSTRACT

Objetivo: relatar um caso de exodontia do dente 43, incluso, na região do mento, com o auxílio de um exame complementar eficaz e de baixo custo: radiografia lateral de mandíbula utilizando filme oclusal. Relato de caso: paciente do gênero masculino, 26 anos de idade, melanoderma e normossistêmico, buscou tratamento ortodôntico por motivos de apinhamento dental anterior e presença do dente 83 (canino inferior decíduo) não esfoliado. Para um planejamento cirúrgico correto, foi realizada a radiografia lateral do mento com filme oclusal, em que se constatou que o dente 43 encontrava-se por vestibular em relação aos dentes anteriores inferiores. Assim, o paciente foi submetido à cirurgia sob anestesia local, para exodontia do dente 43. Considerações finais: o tratamento para transmigração de caninos inferiores deve incluir minuciosos planejamentos clínico e radiográfico. O tratamento sugerido para a grande maioria dos casos é a remoção cirúrgica, devido à possibilidade de falhas dos outros meios de tratamento, ocasionando complicações posteriores. (AU)


Objective: this paper aims to describe and discuss a surgical removal of canine included in the chin region with the aid of an effective and low-cost complementary exam: lateral radiographic of the mandible using the occlusal film. Case report: a male patient, 26 years old, without systemic diseases sought orthodontic treatment due to anterior crowding and presence of an unerupted tooth 83. In order to perform a correct surgical planning, the lateral radiograph of the occlusal film was performed, where it was found that the tooth 43 was in vestibular position in relation to the lower anterior teeth. Thus, the patient underwent surgery under local anesthesia for tooth extraction. Final considerations: the treatment for transmigration of lower canines should include careful clinical and radiographic planning. The suggested treatment for the vast majority of cases is surgical removal due to the possibility of failure of the other means of treatment, causing subsequent complications. (AU)

RESUMEN Introducción: la alveolitis dentaria es una complicación pos-textracción dental, local, dolorosa y reversible del alveolo. Objetivo: caracterizar la alveolitis dental en pacientes adultos del Policlínico "René Bedia Morales", perteneciente al municipio Boyeros. Mareriales y métodos: se realizó un estudio descriptivo de corte transversal, en el Policlínico "René Bedia Morales", municipio Boyeros, provincia La Habana, en el periodo comprendido de septiembre de 2016 a septiembre de 2017. El universo estuvo constituido por 250 pacientes, de ambos sexos. Diagnosticados con alveolitis dental tras haberse realizado exodoncia durante ese periodo. La muestra quedó conformada por 187 pacientes que cumplieron con los criterios de inclusión y exclusión. Se estudiaron las variables: edad, sexo, tipo de alveolitis y factores asociados a las mismas. La información fue recogida en un modelo de recogida de datos, bajo su consentimiento informado. Resultados: el grupo de edad más afectado fue el de 35-59 años, con 47,59 %. Predominó el sexo femenino, con un 66,84 %. Prevaleció la alveolitis seca, en un 55,08 %. Relacionado con las conductas inadecuadas del paciente, la afectación fue de un 40,64 %. El no cumplimiento de las indicaciones postoperatorias y fumar, luego de la exodoncia, fueron los factores más asociados a la alveolitis seca; con un 53,48 % y un 41,18 %, respectivamente. Conclusiones: la alveolitis afectó fundamentalmente a los pacientes de 35-59 años de edad, del sexo femenino. Con mayor frecuencia predominó la alveolitis seca relacionada a las conductas inadecuadas del paciente (AU).


ABSTRACT Introduction: Dental alveolitis is a local, painful dental pos extraction complication which is reversible. Objective: Provide a characterization of dental alveolitis in René Bedia Morales policlinic at Boyeros Municipality. Materials and methods: A descriptive transversal study was performed in René Bedia Morales policlinic at Boyeros Municipality, in Havana Province. The study was performed in a time span between September 2016 to September 2017. The study universe was constituted by 250 patients of both sexes diagnosed with dental alveolitis after have made extractions in that period of time and the sample remained formed for 187 patients , that met with the criteria of inclusion and exclusion. Age, sex, type of alveolitis and associated factors were the variables considered in the present study. The data was collected in a input data form, under the patients concern. Results: The most affected age group was from 35 to 59 years old which represents a 47,59 %, being the feminine sex the predominant group which represents a 66,84 %. Dry alveolitis was predominant in 55,08 % related to inadequate behaviors within a 40,64 % The most frequent factors associated to the non-fulfillment of the pos operatory orders, such as smoking after having performed the procedure of exodontia with a 53,48 % and 41,18 % respectively. Conclusions: Alveolitis mainly affects patients within 35-59 years old, feminine sex, and also related to inadequate behaviors of the patients (AU).

ABSTRACT

síndrome de Lemierre caracteriza-se pela tromboflebite séptica da veia jugular interna, após uma orofaringite, com embolização séptica para o pulmão ou outros órgãos. Neste relato de caso, apresentamos uma paciente feminina, 37 anos de idade, com história de edema e dor em hemiface direita há três dias, associada a fadiga e dispneia progressiva há um dia. História de extração dentária do elemento 48 há três dias. No exame físico admissional, apresentava-se taquipneica, saturando 60% (em ar ambiente), com edema em ângulo da mandíbula direita, redução difusa do murmúrio vesicular e panturrilhas sem empastamento. Angiotomografia de tórax e exames laboratoriais foram compatíveis com quadro de embolia séptica, e tomografia computadorizada da cervical corroborou o diagnóstico de tromboflebite séptica da veia jugular interna. Foi tratada com antibióticos e sintomáticos. A síndrome de Lemierre afeta mais homens jovens e tem embolização para o pulmão em até 97% dos casos. Extrações dentárias raramente podem ser a etiologia dessa síndrome. A tomografia computadorizada é o método de imagem mais utilizado no diagnóstico, e o tratamento é, essencialmente, com antibióticos; portanto, a abordagem cirúrgica é raramente necessária


Lemierre syndrome is characterized by septic thrombophlebitis of the internal jugular vein, after an oropharyngeal infection, with septic embolization to the lungs or other organs. This case report describes a 37-year-old female patient who presented with edema and pain in the right hemiface with onset 3 days previously and progressive fatigue and dyspnea since the previous day. She had had tooth 48 extracted 3 days previously. Physical examination at admission found tachypnea, with 60% saturation (in room air), edema at the angle of the right mandible, diffuse reduction of vesicular murmur, and calves free from clubbing. Angiotomography of the chest and laboratory tests were compatible with septic emboli, and cervical computed tomography confirmed a diagnosis of septic thrombophlebitis of the internal jugular vein. She was managed with antibiotics and given treatment for her symptoms. Lemierre syndrome most often occurs in young men and there is embolization to the lungs in up to 97% of cases. Rarely, the etiology of this syndrome may be tooth extraction. Computed tomography is the imaging method most often used for diagnosis and treatment is basically antibiotic. Surgery is thus rarely necessary

ABSTRACT

Dental transplantation is a treatment used to replace a missing tooth with another tooth. It is autogenous when it comes from the same individual. The aim of this article is to discuss the surgical protocol and present a case study to optimize the technique for the general dental practitioner. The first molars are the first permanent teeth to erupt during infancy, they are teeth most commonly lost among the permanent dentition. The third molars are best suited for this treatment, the treatment should be undertaken as soon as possible, as the patient ages there is decrease in the mesenchymal cells. Whenever possible, the pericoronal bag should be preserved in the removal of the tooth germ or tooth formed. We present a case where the tooth was placed in the receiver socket, taking into account the anatomical orientation of its faces, in infra-occlusion. The contention was made by silk and point "X" on the occlusal graft and fixed to the adjacent teeth with light-cured resin. Transplantation is an excellent alternative for treating the loss of one molar, especially in young patients.

Resumo O estudo objetivou criar um instrumento de medida que pudesse quantificar os desconfortos associados ao procedimento cirúrgico de pacientes submetidos a cirurgias dento-alveolares (QCirDental). O instrumento QCirDental foi desenvolvido em duas etapas iniciais: (1) geração e seleção das perguntas ou itens e, (2) teste do instrumento com avaliação das propriedades de medida (consistência interna e responsividade). A amostra foi composta por 123 pacientes. Nenhum deles apresentou qualquer dificuldade em entender o QCirDental. O instrumento mostrou excelente consistência interna com coeficiente alfa de Cronbach de 0,83. A análise de componentes principais (Kaiser-Meyer-Olkin Measure of Sampling Adequacy 0,72 e Bartlett's Test of Sphericity com p < 0,001) mostrou seis (6) dimensões do instrumento que explicam 67,5% da variância. O QCirDental possui excelente consistência interna, sendo um instrumento de fácil leitura e interpretação compatível com uma adequada validade semântica e de conteúdo. Mais de 80% dos pacientes submetidos a extrações dentárias de rotina odontológica apresentam algum desconforto ou incomodo no período transoperatório o que ressalta a necessidade de utilização de um instrumento para avaliação de cuidados.


Abstract The scope of this study was to develop and validate a questionnaire (QCirDental) to measure the impacts associated with dental extraction surgery. The QCirDental questionnaire was developed in two steps; (1) question and item generation and selection, and (2) pretest of the questionnaire with evaluation of the its measurement properties (internal consistency and responsiveness). The sample was composed of 123 patients. None of the patients had any difficulty in understanding the QCirDental. The instrument was found to have excellent internal consistency with Cronbach's alpha reliability coefficient of 0.83. The principal component analysis (Kaiser-Meyer-Olkin Measure of Sampling Adequacy 0,72 and Bartlett's Test of Sphericity with p < 0.001) showed six (6) dimensions explaining 67.5% of the variance. The QCirDental presented excellent internal consistency, being a questionnaire that is easy to read and understand with adequate semantic and content validity. More than 80% of the patients who underwent dental extraction reported some degree of discomfort within the perioperative period which highlights the necessity to assess the quality of care and impacts of dental extraction surgery.

Abstract This study evaluated the effects of hyaluronic acid (HA) on bone repair of human dental sockets. Thirty-two lower first premolars were extracted from 16 patients (2 per patient) for orthodontic reasons. Following the extractions, one socket was randomly filled with 1% HA gel, while the other was allowed to naturally fill with blood clot. After 30 and 90 days of surgery, patients underwent cone beam computed tomography. Five central orthoradial slices were captured from each socket. The gray intensity was measured in each image and results were reported as mean percentage of bone formation. The buccolingual alveolar ridge width was measured and dimensional changes were compared between the postoperative intervals. The pattern of alveolar trabecular bone was evaluated through the fractal dimension. Treated sockets showed a higher percentage of bone formation and fractal dimension values (58.17% and 1.098, respectively) compared with controls (48.97% and 1.074, respectively) in the 30-day postoperative period (p < 0.05). After 90 days, there was no significant difference between groups. Additionally, no significant difference was found between groups regarding the alveolar dimensions (p > 0.05). Use of 1% HA gel after tooth extraction accelerates bone repair in human dental sockets.

Premolars are the most commonly extracted teeth to provide space to correct crowding and excessive labial protrusion. After treatment, the extraction spaces have to remain closed. Nevertheless, several studies have shown that there is a tendency for some relapse even in patients finished with an adequate occlusion. The objective of this study was to compare the stability of extraction space closure of the first and second premolars. A sample 72 patients´ dental casts were divided into two groups. Group 1, comprised 29 patients (116 extraction spaces) were treated with first premolar extractions at a mean initial age of 13.78 years and group 2, comprised 43 patients (100 extraction spaces) were treated with second premolar extractions at a mean initial age of 15.20 years. The dental casts obtained at pretrement, posttreament and a between 3 to 4 years postretention were digitized using a 3- dimensional scanner (R700; 3Shape,Copenhagen, Denmark). Chi-Square tests were used to compare the numbers of open and closed extraction spaces after treatment and at long-term posttreatment. T tests were used to compare the amount of spaces at posttreatment and at the long-term posttreatment stages. These tests were also performed in subgroups with completely closed extraction sites at posttreatment. The groups showed similar numbers of extraction sites reopening. First and second premolar extraction space closure present a similar tendency for reopening. Considering only the cases that showed completely closed extraction spaces in the final dental models, extraction space reopening was larger in the first premolar extraction group in the maxillary arch.(AU)


Os pré-molares são os dentes mais comumente extraídos para corrigir o apinhamento dentário e à protrusão labial excessiva. Após o tratamento, os espaços das extrações deveriam permanecer fechados. Contudo, muitos estudos demostraram que existe uma tendência à reabertura dos espaços de extrações em pacientes finalizados com uma oclusão adequada. O objetivo deste estudo foi comparar a estabilidade dos espaços de extrações de primeiro e segundo prémolares. A Amostra deste estudo foi composta por 72 modelos dentários dividido em dois grupos. O Grupo 1 composto por 29 pacientes (116 espaços de extração) foram tratados com extrações dos primeiros pré-molares com idade media inicial de 13,78 anos e o grupo 2 composto por 43 pacientes (100 espaços de extração) foram tratados com extrações dos segundos pré-molares com idade media inicial de 15.20 anos. Os modelos dentários obtidos no pré-tratamento, pós-tratamento e 3 a 4 anos de controle e foram digitalizados mediante um scanner 3Shape R700 3D (3Shape A/S, Copenhagen, Dinamarca). Os testes t e do Qui-Quadrado, foram utilizados para comparar o número de espaços de extração abertos e fechados após o tratamento e pós-tratamento em longo prazo. Os resultados demostraram números similares de reabertura do espaço de extração entre os grupos. Concluiu-se que considerando apenas os casos que mostraram espaços de extração completamente fechados no final do tratamento, a quantidade de reabertura dos espaços de extrações dos primeiros pré-molares ocorre mais frequentemente que dos segundos pré-molares no arco superior.(AU)
